Acute mesenteric lymphadenitis is more common in the terminal ileum and is often caused by hematogenous infection of streptococci. It is also believed to be related to intestinal inflammation and parasitic diseases. The lymph nodes are multiple and congested and swollen. There may be a small amount of inflammatory exudate in the abdominal cavity. Microscopically, the lymphatic sinuses were dilated and neutrophils entered the lymphatic sinuses from small blood vessels and phagocytosed bacteria. Some white blood cells may degenerate and collapse as a result, forming cell fragments or denatured substances. The blood vessels in the lymph nodes also dilate and become congested, the germinal centers proliferate, and the sinus cells and immunoblasts proliferate. Acute mesenteric lymphadenitis is more common in children under 7 years old. Before the onset of the disease, there are often prodromal symptoms such as sore throat, fever, fatigue and discomfort, followed by umbilical and right lower abdominal pain, nausea, vomiting, and sometimes diarrhea or constipation. This disease course is exactly the opposite of acute appendicitis, which is characterized by abdominal pain followed by fever, and the body temperature rises sharply in the early stage of the disease. During physical examination, there may be tenderness in the navel and right lower abdomen, which is relatively widespread and the tenderness point is not fixed. Because children's abdominal muscles are underdeveloped, abdominal muscle tension may not be obvious.

Acute nonspecific mesenteric lymphadenitis should be treated with non-surgical methods, and the decision to perform surgery should be made with caution.

1. Non-surgical treatment

When considering this disease, active conservative treatment should be given for 24 to 48 hours. If the diagnosis is clear and the measures are appropriate, the abdominal pain symptoms and signs can generally be gradually alleviated and improved within 24 hours. The specific measures are:

(1) Rest in bed, refrain from eating or drinking, closely monitor changes in body temperature and blood count, and pay attention to the development of abdominal symptoms and signs

(2) Intravenous rehydration to correct water and electrolyte imbalance.

(3) Use broad-spectrum antibiotics to quickly control the infection. Traditional Chinese medicine detoxifying and heat-clearing agents also have a certain effect.

(4) Abdominal physical therapy to promote the limitation and absorption of inflammation.

2. Surgery

If the condition does not improve after active non-surgical treatment, or if other acute abdominal diseases are considered to be present, and if acute appendicitis cannot be ruled out, surgical treatment should be performed decisively:

(1) Epidural anesthesia or general anesthesia is used through a longitudinal incision in the right lower abdomen.

(2) Appropriate exploration includes the ileocecal region, appendix, terminal ileum and its mesentery. Female patients should also have the ovaries and appendages explored to confirm the diagnosis. For suppurative mesenteric lymphadenitis, abdominal drainage is often done; when adjacent intestine is involved, resection of the affected intestine is sometimes required.
