1. Degeneration of the cervical spine

Degenerative changes of the cervical spine are the main cause of cervical spondylosis, among which the degeneration of the intervertebral disc is particularly important and is the primary factor in the degeneration of the cervical spine structures. From this, a series of pathological anatomical and pathophysiological changes of cervical spondylosis have evolved.

(1) Intervertebral disc degeneration: When the intervertebral disc begins to degenerate, it loses its normal function due to changes in its morphology, which in turn affects or destroys the biomechanical balance of the cervical motion segment and causes a series of changes in related structures. Therefore, the degeneration of the cervical intervertebral disc becomes the main factor in the occurrence and development of cervical spondylosis.

(2) The emergence of the ligament-disc space and the formation of hematoma are crucial to the occurrence and pathogenesis of cervical spondylosis, and are also the pathological anatomical basis for the transition from cervical disc disease to osteogenic cervical spondylosis. In fact, in the early stages of cervical spondylosis, due to the degeneration of the intervertebral disc, not only does the dehydrated and hardened nucleus pulposus gradually shift to the back or front of the vertebra, and finally protrude under the ligament, thereby increasing the local pressure and causing the separation of the ligament together with the periosteum and the surrounding cortical bone of the vertebral body, but the degeneration of the intervertebral disc itself can also cause loosening and abnormal movement of the intervertebral joints, thereby further aggravating the tearing of the ligament and periosteum and accelerating the formation of the ligament-disc space.

The gap formed after the separation of the ligaments behind the intervertebral disc is often accompanied by local microvascular tearing and bleeding, forming a ligament-disc space hematoma.

(3) Bone spur formation at the edge of the vertebral body As a hematoma forms in the subligamentous space, fibroblasts begin to become active and gradually grow into the hematoma, eventually replacing the hematoma with granulation tissue. As the hematoma organizes, ossifies and calcium salts are deposited, osteophytes are eventually formed that protrude into the spinal canal or the front edge of the vertebral body.

(4) Degeneration of other parts of the cervical spine Degeneration of the cervical spine is not limited to the intervertebral disc and the adjacent vertebral margins and uncovertebral joints, but should also include: ① Small joints often degenerate after the intervertebral disc degeneration causes instability and abnormal activity of the intervertebral joints. ②The yellow ligament often begins to degenerate based on the degeneration of the first two. Its early manifestation is ligament relaxation, which gradually proliferates and thickens, and protrudes into the spinal canal. Calcification or ossification may occur in the later stages. ③ The degenerative changes of the anterior longitudinal ligament and the posterior longitudinal ligament are mainly manifested as fibrous hyperplasia and sclerosis of the ligament itself, and in the later stage, calcification or ossification is formed, which is consistent with the diseased vertebrae.
