Cause: Hypertrophy of nasal concha in children usually develops from chronic simple rhinitis. The cilia of the mucosal epithelium fall off and become stratified cuboidal epithelium. Edema and subsequent hyperplasia of fibrous tissue occur in the submucosal layer, causing mucosal hypertrophy. Over time, it may become mulberry-shaped or polyp-like, with hyperplasia of periosteum and bone tissue, and hypertrophy of the nasal concha bones. Treatment: (i) The use of vasoconstrictor nasal drops is limited to mild cases. (ii) Submucosal sclerosing agent injection of the inferior turbinate. Its mechanism of action is that after the sclerosing agent is injected, a local chemical inflammatory reaction can occur, producing scar tissue, reducing the volume of the turbinate and improving ventilation. (iii) Submucosal electrocoagulation of the thickened mucosal tissue under the inferior turbinate causes the scar to shrink. (iv) Cryosurgery involves placing a specially made cryotherapy head on the surface of the inferior turbinate to freeze the area for 1 to 2 minutes each time, causing the diseased mucosa to necrotize and fall off, thereby regenerating the mucosa. (V) Surgical treatment: If general treatment is ineffective, or the mucosa is significantly thickened, or the thickened part is located at the posterior end or lower edge of the inferior turbinate, partial resection of the inferior turbinate or partial resection of the middle turbinate can be performed. The inferior turbinate should not be removed too much. In principle, it should not exceed 1/3 of the inferior turbinate, so as not to affect the function of the nasal mucosa or cause secondary atrophic rhinitis. For patients with bony hypertrophy, inferior turbinate muco-subperiosteal resection is recommended, which can improve nasal ventilation and drainage without damaging the physiological function of the nasal mucosa. (VI) Appropriate treatment shall also be given to chronic systemic diseases or adjacent lesions such as deviated nasal septum or sinusitis. Plasma cryoablation treatment: For nasal concha hypertrophy, Chinese patent medicine can be the first choice, and the compound Tongqiaozhi.liusan can be used for conservative treatment with good results. If drug treatment is ineffective, surgical treatment can be considered, such as DNR cryogenic plasma ablation. Under the action of low temperature, it can ablate the lesions of turbinate hypertrophy without damaging the normal turbinate mucosal tissue, thus achieving the treatment goal [1]. The working principle of the plasma cryoablation treatment system is to form a thin plasma layer between the electrode and the tissue. The ions in the layer are accelerated by the electric field and transfer energy to the tissue. At low temperatures (40℃~70℃), the molecular bonds between cells are opened, and the cells in the target tissue are decomposed into carbohydrates and oxides, causing liquefaction and ablation of the diseased tissue, which is called plasma (not thermal effect), thereby achieving the effect of reducing the volume of scar tissue.
