Materials that the adopter should submit (application materials) (1) Adoption Application (2) Application form for whether you have children (obtained from the Family Planning Office) (3) A certificate issued by the GA agency reporting the discovery of an abandoned baby or child (4) Publish a notice in the local county newspaper to search for the child (5) A health certificate issued by a county-level or higher medical institution that shows that the child does not suffer from any disease that medically prohibits adoption. (6) Infertility certificate issued by a medical institution at or above the county level (7) Household registration booklet and a copy of the marriage certificate; if the adopter is not married, a certificate of not being married issued by the village (neighborhood) committee where the adopter lives (8) One family photo and one individual photo (2 inches) (9) Annual family income certificate issued by the adoptive employer or neighborhood (village) committee 2. Adoption registration procedures: Application - Acceptance - Review - Registration (Issuance of Certificate) If a couple jointly adopts a child, they must jointly go through the registration procedures at the adoption registration authority. The adoptee who is 10 years old or above must sign his/her opinion. When accepting an adoption registration application, the adoption registrar shall follow the procedure of reviewing the materials - filling out the "Adoption Registration Inquiry Record" - filling out the "Adoption Registration Application Form" and having the adopter and adoptee sign and fingerprint - registration (issuance of certificate). |
