There are often some loose and easy-to-fall-off hair at the edge of the hair loss area, some of which have been broken, and the hair at the proximal end often atrophies. If the hair is pulled out, you can see that the hair is thick on the top and thin on the bottom, like an exclamation mark, and the hair pigment at the bottom is also lost. This phenomenon is a sign of progression. The hair loss continues to increase, and each piece also expands and can merge with each other to form irregular shapes. If it continues to progress, it may be completely bald. In severe cases, eyebrows, eyelashes, armpit hair, pubic hair and body vellus hair also fall off, which is called universal alopecia.

Hair loss can also stop. At this time, the area of ​​hair loss will no longer expand, and the hair on the edges will be firmer and not easy to pull out. After several months, the hair will grow gradually or rapidly. Some patients will first grow white hair, which will then gradually become thicker, darker, and longer and become normal hair.

The scalp of a person with hair loss is normal, smooth, and non-inflamed. Sometimes it looks thinner and slightly concave. This is due to the disappearance of hair and hair roots, not actual thinning of the scalp.
