What should be paid attention to in diagnosing chickenpox

(1) Impetigo often occurs around the nose and lips or exposed parts of the limbs. It is considered as herpes, which then develops into pustules and then scabs. It does not appear in batches, is not seen in mucous membranes, and has no systemic symptoms.

(ii) Papular urticaria is a type of spindle-shaped edematous red papules, the size of a peanut, with a needle-tip or millet-sized papulovesicle or blister in the center, which is very itchy. Distributed on the limbs or trunk, not involving the head or mouth, and no crusting.

(III) Herpes zoster: The herpes zoster is distributed along a certain nerve trunk path, is asymmetrical, does not exceed the midline of the trunk, and has significant local burning pain.

(V) Other viral infections Herpes simplex virus infection can also cause varicella-like skin lesions. This type of disseminated herpes simplex virus infection is often secondary to skin diseases such as atopic dermatitis or eczema, and the diagnosis depends on the results of virus isolation. In recent years, it has been found that enterovirus, especially Coxsackievirus group A, can cause widespread varicella-like rash, which usually occurs in late summer and early autumn when enterovirus is prevalent, and is often accompanied by lesions in the throat, palms, and soles of the feet. This helps to distinguish varicella from enterovirus infection.

Ways to prevent chickenpox

(1) Passive immunity

Intramuscular injection of 5mL of high-titer VZV immune globulin within 72 hours after exposure has a preventive effect on chickenpox. Due to the inherent safety issues of blood products, the application of VZV immune globulin is limited to the following situations exposed to varicella patients: those with cellular immunodeficiency, those receiving long-term immunosuppressant treatment, and those with serious diseases such as leukemia, lymphoma and other malignant tumors. The reason is that once such patients are infected with VZV, they may develop severe varicella, the severity of which will outweigh certain risks associated with transfusion of blood products.

(2) Autoimmunity

In recent years, inactivated VZV vaccines or live attenuated vaccines have been tried, mainly for people at high risk of varicella. They have a certain preventive effect and are said to have protection that can last for more than 10 years.
