1. Topical medications for oral candidiasis

1.1. 2% to 4% sodium bicarbonate (baking soda) solution: This medicine is commonly used to treat thrush in infants and young children. It is used to wash the mouth before and after breastfeeding to remove residual curd or sugar that can decompose and produce acid, making the mouth alkaline environment and preventing the growth and reproduction of Candida albicans. Children with mild symptoms do not need other drugs, and the lesions will disappear within 2 to 3 days, but they still need to continue taking the medicine for several days to prevent recurrence. This medicine can also be used to clean the nipples before and after breastfeeding to avoid cross-infection or repeated infection.

1.2. Gentian violet (gentian violet) aqueous solution: Gentian violet solution can still inhibit the growth of Candida albicans at a concentration of 1:100,000. The best concentration for oral mucosa is 1/2000 (0.05%), applied three times a day to treat thrush and angular cheilitis in infants and young children. However, after drug staining, it is not appropriate to observe changes in damage. Commercially available 1% methyl purine solution. Due to its high irritation, it should not be used directly on the oral mucosa of infants and young children, but it can be used for skin lesions.

2. Antifungal drugs for oral candidiasis

Mycostatin: This drug belongs to the tetraene antibiotics, 1 mg is equivalent to 2000 U and should be stored at low temperature. It is not easily absorbed by the intestines, so it is often used to treat Candida albicans infections of the skin, mucous membranes and digestive tract. 50,000 to 100,000 U/ml of aqueous suspension can be applied topically once every 2 to 3 hours and can be swallowed after application. You can also use gargles to rinse your mouth, or make them into lozenges, emulsions, etc. Children (1-2 years old) should take 100,000 U/time orally, 3 times a day; adults should take 500,000-1,000,000 U/time orally, 3 times a day. The antibacterial effect of this drug may be achieved by destroying the cell membrane and releasing potassium, thereby causing the cessation of glycogenolysis in the cell and causing the cell to lose vitality. Oral side effects are minimal, occasionally causing nausea, diarrhea, or loss of appetite. The course of treatment is 7 to 10 days.

3. Comprehensive treatment of oral candida infection

For patients with obvious congestion and edema of the mucosa, red tongue, bad breath, yellow urine, constipation and other symptoms of heart fire inflammation or stomach heat and dampness, "Stomatitis Granules" (see the chapter on herpes simplex) can be taken.

In addition to antifungal drugs, patients with weak bodies, immunodeficiency diseases or related systemic diseases and chronic Candida infections often need comprehensive treatment measures to enhance the body's immunity, such as injections of transfer factor, thymosin, lipopolysaccharide, iron supplements, vitamin A, and multiple small-scale blood transfusions.

Tests for oral candidiasis

The most reliable laboratory diagnosis method for candidiasis is currently considered to be the formation of thick-walled spores on corn culture medium, while the simplest method is direct microscopic examination of specimens.

Dentists often take specimens of pseudomembranes, desquamated epithelium, and scabs from the oral mucosa, place them on a slide, add a few drops of 10% potassium hydroxide solution, cover with a cover slip, heat over a low flame to dissolve the keratin, and then immediately perform a microscopic examination. If pseudohyphae or spores are found, it can be confirmed as a fungal infection, but culture is still necessary to confirm that it is Candida albicans.

Acute pseudomembranous Candidal stomatitis should be differentiated from acute coccal stomatitis (membranous stomatitis). Membranous stomatitis is caused by infection with cocci such as Staphylococcus aureus, hemolytic Streptococcus, and Pneumococcus. It is common in children and the elderly and can occur in any part of the oral mucosa. The affected area is obviously congested and edematous, and a large amount of fibrinogen exudes from the blood vessels and coagulates into a grayish white or grayish yellow pseudomembrane with a smooth and dense surface that is slightly higher than the mucosal surface. The pseudomembrane can be easily wiped off, leaving behind an eroded surface with bleeding. Regional lymphadenopathy may be accompanied by systemic reactions. Smear examination or bacterial culture can identify the main pathogens.

Nursing care for oral candidiasis

1. Nursing:

1.1. Reasonable nutrition: Food should be as diverse as possible, with a balanced combination of coarse and fine grains. Eat more beans and mushrooms.

1.2. Rich in nutrition: Eat more nutritious, easy to absorb and digest foods, and eat more high-protein, multi-vitamin, and low-animal fat foods.

1.3. Fresh and light diet: Do not eat stale, spoiled or irritating food; it is recommended to eat more fresh fruits and vegetables rich in vitamin C; choose light-tasting, easily digestible food to keep bowel movements smooth; you can drink more yogurt, mushroom soup, kelp soup, and yellow croaker soup.

2. Diet therapy:

2.1. Add 5 grams of sesame oil into light salt soup, wait until it is cool, then drop it into the child's mouth, 4-5 drops each time, more than 10 times a day. This prescription has the effect of clearing heat and moistening dryness.

2.2. Wash 90-120 grams of wild rose flowers, steam them, take the water from the lid of the pot, condense it and collect it in a bottle for later use. Use a cotton swab to dip this liquid and gently wipe off the pseudomembrane in the child's mouth, then apply it to the affected area. This prescription has the effects of clearing away heat, detoxifying and promoting healing.

2.3. Take 1 small cup of honeysuckle dew for children, drink it 3 times a day. Or wipe the false membrane with honeysuckle dew. This prescription has the effect of clearing heat and purging fire.
