Fetal hypoxia can cause group reflexes, excretion of meconium, and true breathing, inhaling meconium, amniotic fluid and keratinized cells together. Inhalation of large amounts of thick meconium can cause complete tracheal obstruction, atelectasis, and acute asphyxiation and hypoxia. Thin meconium and amniotic fluid or small amounts of inhaled fluid can lead to partial obstruction, causing subsegmental atelectasis and obstructive emphysema. If the alveoli rupture, it can cause interstitial emphysema or pneumothorax. When gas spreads along blood vessel walls and lymphatic vessels to the mediastinum, it can cause mediastinal emphysema.

Meconium and amniotic fluid irritation to the respiratory mucosa or secondary infection can lead to pneumonia. After entering the recovery period, the inhaled substances are absorbed or swallowed, and in severe cases, fibrosis and lesions such as emphysema may form.

At birth, the whole body is stained with meconium. If the fetus is immersed in meconium and amniotic fluid for 4 to 6 hours, the fingernails and toenails may be stained yellow-green. If it is 10 to 12 hours, the umbilical cord, vernix caseosa, and placenta and amniotic membrane may all be stained with meconium color. The newborn's breathing is severely suppressed, and he or she may experience bradycardia, low muscle tone, and shock. After spontaneous breathing, shortness of breath may be observed, gradually leading to difficulty breathing, intercostal retractions, expiratory groans, and cyanosis of the skin. If the cyanosis is severe and does not improve with oxygen inhalation, persistent pulmonary hypertension of the newborn should be considered. Pulmonary signs are related to the amount and thickness of meconium inhaled. If there is pneumothorax, the breathing movements and breath sounds on both sides may be asymmetric.

Severe asphyxia and hypoxia can cause cardiovascular adaptive disorders, right-to-left shunt, megacardia with poor peripheral circulation congestion, and occasional convulsions. Blood gas analysis showed decreased PO2, increased Pco2 and decreased pH. Severe cases die within minutes to hours after birth, while survivors have a prolonged illness. Generally, cases show significant improvement within 2 days and complete recovery after 10 days or longer. In mild cases, breathing difficulties may not occur until several hours after birth. X-ray examination shows slight thickening of lung texture, mild emphysema, and slight descent of the diaphragm. For those who have inhaled more meconium, both lungs may have dense patchy or nodular infiltration, excessive lung expansion, occasional small amount of pleural effusion, interstitial emphysema, mediastinal gas or pneumothorax.
