3. Bleeding: Peptic ulcer bleeding is the most common cause of upper gastrointestinal bleeding, accounting for 1/3 to 1/2 of it. Patients with bleeding duodenal ulcer have no specific symptoms, and only 30% to 40% of bleeding patients have early ulcer symptoms. People with duodenal ulcers are often debilitated, sometimes with anemia and weakness due to chronic blood loss from the ulcer.

Some cases of duodenal ulcer may not have typical pain, but only manifest as irregular and vague upper abdominal pain and discomfort, accompanied by symptoms such as abdominal distension, anorexia, and belching. As the disease progresses, symptoms may change due to the emergence of complications. Generally speaking, duodenal ulcer is characterized by upper abdominal pain in an uncertain location. If the pain worsens and is located in a fixed location, radiates to the back, and cannot be relieved by antacids, it often indicates chronic perforation of the posterior wall. Acute perforation should be considered when severe abdominal pain occurs suddenly and spreads rapidly to the entire abdomen. Sudden dizziness indicates possible bleeding complications.
