The thickness of the tunica albuginea can normally be up to 2 mm, but after erection it is only 0.25-0.5 mm thick, so it is easy to rupture. Once ruptured, blood spurts out from the corpus cavernosum into the subcutaneous tissue of the penis, accompanied by severe pain. Immediate surgery is often required, otherwise the swelling will be quite severe. First, the penis should be fixed and elevated, cold compress should be applied to stop the bleeding, and the ruptured tunica albuginea should be sutured, but the penis will often be left bent in the opposite direction of the fracture and suture. Penile Trauma: Contusion

It is mostly caused by the direct action of blunt external force when the penis is erect.
