Baby has coffee spots on face?

Many babies have coffee spots on their faces in life. In fact, coffee spots on babies' faces are mostly light brown patches found at birth. Coffee spots are not affected by sun exposure, and the surface skin of most coffee spots is normal. The formation of baby coffee spots is mostly a hereditary skin disease. Parents should pay attention to it. If the baby has coffee spots, parents must take the baby for examination and treatment in time.

Symptoms of coffee stains

Light brown patches can be found at birth, ranging in color from light brown to dark brown, but each patch is the same color and very uniform, and the depth is not affected by sunlight. The size ranges from a few millimeters to tens of centimeters, with clear boundaries and the surface skin texture is completely normal. The symptoms are very similar to freckles, mainly characterized by an abnormal increase in the amount of melanin in the epidermis, but the number of melanocytes is normal.

Café au lait macules are most common in patients with neurofibromatosis. About 70% of patients with neurofibromatosis have café au lait macules. If there are six café au lait macules with a diameter greater than 1.5 cm, it has important diagnostic reference value. However, most patients with cafe au lait spots are not patients with neurofibromatosis. Cafe au lait spots are simply a manifestation of congenital hyperpigmentation of the epidermis. In addition, cafe au lait spots can also be seen in tuberous sclerosis and other neuroectodermal syndromes.

Differential diagnosis of café au lait spots:

1. Pigmented spots: Mixed nevus is a nevus in which nevus cells and nevus cells are nested in the deep layer of the epidermis and also in the dermis. Common moles have a central bulge that protrudes from the skin and is covered with hair, and are diffusely distributed around the mole with different colors and unclear boundaries. They are components of junctional nevi. Junctional nevi transform into intradermal nevi in childhood, so mixed nevi are rare in the elderly. Both compound nevi and junctional nevi have the potential to become malignant.

2. Skin mottled pattern: Skin mottled pattern is one of the clinical symptoms of sporadic cretinism. During the neonatal period, the patient may have persistent jaundice, difficulty sucking, constipation, difficulty breathing, snoring (nasal congestion, thick tongue), hoarseness, weak crying, little movement, abdominal distension and skin mottling.

3. Skin spots: Dark spots can be divided into two types: congenital and acquired.

The treatment results of café au lait spots are sometimes difficult to predict due to the active metabolism of local melanocytes. The average number of treatments for café au lait spots is relatively high, and some lesions may develop reactive pigmentation after treatment, making it difficult to continue treatment. It is necessary to wait for the reactive pigmentation to subside naturally before continuing treatment. Generally, there is no hypopigmentation or loss of pigmentation, change in skin texture, or scar formation after the operation.

Since the melanocytes of café au lait spots are distributed in the epidermis, the epidermal melanocytes can be destroyed by using shorter wavelengths and smaller energy density. It is generally believed that a wavelength of 504nm is an ideal choice with less non-specific damage. Therefore, a 510nm pulsed dye laser and a 532nm Q-switched frequency-doubled Nd:YAG laser are both good choices.
