What are the types of cervical tuberculosis in children?

Classification of lymph node tuberculosis: caseous tuberculosis, proliferative tuberculosis, mixed tuberculosis, and anergic tuberculosis.

Cervical tuberculosis develops into four types according to its course:

1) Nodular type: The disease onset is slow. In the early stage, 1-2 progressive painless lumps may appear on one or both sides. The lumps are slightly hard, scattered and movable, and have no adhesions. There is slight pain or tenderness. As the disease progresses, lymphadenitis forms and adhesions appear like beads.

2) Infiltrative type. There is obvious perilymphadenitis, which is adhered to the surrounding tissues, has poor mobility, and is accompanied by conscious pain and tenderness. Highly swollen lymph nodes can be felt.

3) Abscess type: The center of the swollen lymph nodes gradually softens and forms cold abscesses. When accompanied by secondary infection, the pain intensifies and the local area becomes red, swollen, hot and painful, showing symptoms of acute inflammation. Even during chemotherapy, abscesses may occur in 20%.

4) Ulcer-fistula type: The abscess ruptures on its own and does not heal for a long time, forming a fistula or ulcer.

Treatment principles for cervical tuberculosis in children:

1. Pay proper attention to nutrition and rest.

2. Systemic anti-tuberculosis treatment.

3. Treatment of local tuberculous lymph nodes. A small number of localized, larger, and movable lymph nodes can be removed surgically. For those with cold abscesses, ulcers or sinus tracts, the pus can be drained and anti-tuberculosis drugs can be injected into the cavity, or the tuberculosis lesions can be removed and the dressings replaced with anti-tuberculosis drugs and wait for the wound to heal. If necessary, surgical removal can also be performed.
