Treatment of scleredema infantum

1. Rewarming

It is the first step in treatment. Children with mild symptoms should be wrapped in a pre-heated quilt after a warm water bath, placed in a warm room at 24-26℃, and equipped with an external hot water bag. The water temperature should be gradually increased from 40℃ to 60℃, and the body temperature can quickly rise to normal. Children with moderate and severe symptoms can be placed on an open far-infrared warming bed first, and the temperature can be adjusted to 1.5 to 2°C higher than the child's body temperature. The body temperature can be raised by 1°C approximately every 30 minutes. The temperature of the warming bed can be continued to be increased as the child's body temperature rises. When the body temperature reaches 4°C, it can be moved to a closed warming box and the box temperature should be kept at around 5°C. To reduce heat loss due to radiation, cover the child's body with a transparent plastic sheet slightly away from the child's body. Leave the head and face exposed outside the plastic sheet and wear a small hat on the head to keep warm. In addition to the above methods, other methods of rewarming include warm water bath and warm salt water enema. If you are using intravenous fluid supplements or high-nutrient solution, you can wrap a hot towel around the bottle to keep the liquid entering the body at a certain temperature. The oxygen supply must also be preheated.

2. Nutrition and fluids

To ensure adequate supply of calories and fluids, the calories should at least meet the needs of basal metabolism at the beginning, and then gradually increase to normal requirements. Because heart and kidney functions decrease at low temperatures, the amount of infusion should not be too much. For children with hypoglycemia, appropriately increase the amount of glucose intake.

3. Medication

(1) Cardiovascular active drugs such as dopamine and dophenolamine can be given to patients with poor heart and kidney function, because small doses can dilate the kidneys and cerebral blood vessels and increase urine output. Dopamine increases myocardial contraction but does not increase heart rate, and can be used in combination with dopamine. Other drugs such as 654-2 can also be used. If the complexion and heart rate improve, intravenous drip can be used to maintain the condition and treatment can be continued for one week.

(2) The use of antibiotics is particularly important for scleredema caused by infectious diseases, and drugs that are highly toxic to the kidneys should be used as little as possible. Although respiratory infections may occur in patients with cold injury syndrome, broad-spectrum antibiotics should not be used to prevent them.

(3) After heparin treatment is completed until the prothrombin time and coagulation time return to normal, the frequency of administration should be gradually reduced. One course of treatment is 7 days.

(4) Genetic testing: Genes are inherited from parents and remain almost unchanged throughout life. However, due to genetic defects, some people are naturally susceptible to certain diseases. In other words, the presence of certain genotypes in the human body increases the risk of developing certain diseases. Such genes are called disease susceptibility genes.
