Surgical procedures

1. Lie flat on your back.

2. Cleaning and disinfection: Clean the area with soapy water and saline, and disinfect with 1:1000 Sanisol solution; for those with phimosis, use a syringe connected to the venous incision needle to inject Sanisol solution into the foreskin sac for disinfection.

3. Separation and adhesion: patients with narrow foreskin opening and adhesion between foreskin and glans penis. First use a hemostatic forceps to enlarge the foreskin opening, and then use two hemostatic forceps to clamp the middle part of the dorsal edge (the two forceps are 0.2 cm apart). Use a slotted probe to separate the adhesions until the glans penis is completely separated from the foreskin. Then clean the foreskin sac and glans penis with sterile saline.

4. Design the incision and use a hemostatic forceps to clamp the frenulum of the foreskin to lift the foreskin. Use the tip of the knife to make a cut on the outer plate of the foreskin 0.5 cm distal to the edge of the coronal sulcus as a preparation for the circumcision incision, being careful to avoid excessive removal.

5. Use scissors to cut the inner and outer layers of the foreskin along the probe groove for dorsal incision. The inner layer of the foreskin should also be cut to about 0.5 cm away from the edge of the coronal sulcus.

6. Remove the foreskin and align the inner and outer plates of the foreskin. Pull outward the hemostatic forceps clamped on the back of the foreskin and the frenulum, and then check whether the incision on the outer plate of the foreskin is appropriate as a circumcision incision. If appropriate, use curved scissors to cut the right skin flap along the incision about 0.5 cm from the coronal sulcus, and then cut the left side. The inner and outer plates at the frenulum of the foreskin can be left intact, or more of them can be retained.

7. Use fine silk thread to suture once on the back, abdomen, left and right sides of the circular incision. Do not tie the ligature too tightly to avoid damaging the skin when the tissue is edematous. The sutures are not cut short and are reserved for fixing the dressing. Then sew 1-2 stitches between every two sutures, and the needle should be passed close to the incision edge.

8. Bandage: Wrap a piece of Vaseline gauze (with the raw edges folded inside) around the foreskin incision, secure it with long sutures, and then bandage it with several layers of gauze.
