In thalassemia, red blood cell fragility is often reduced; in hereditary spherocytosis, red blood cell fragility is increased; in autoimmune hemolytic anemia or neonatal hemolytic anemia, the anti-human globulin test (Coombs Test) is positive. 3. Liver function tests (1) Serum bilirubin determination: 1-minute bilirubin (1'B) is equivalent to conjugated bilirubin (the bilirubin content measured 1 minute after the serum and reagent are mixed), which generally accounts for 20% of the total bilirubin (TB). In hemolytic jaundice, the ratio of l'B/TB is generally less than 20%; in hepatocellular jaundice, the ratio of l'B/TB is often between 20% and 60%; in obstructive jaundice, the ratio is often greater than 60%. The experimental identification of bilirubin metabolism of the three types of jaundice is shown in Table 1. (2) Serum protein determination and protein electrophoresis: In the middle and late stages of hepatocellular jaundice, serum total protein and albumin decrease, globulin increases, and the albumin/globulin ratio decreases or even inverts (the normal ratio is 2:1). In rare cases, such as when globulin is significantly elevated, total protein may be normal or above normal. Protein electrophoresis showed that in patients with acute icteric hepatitis, beta and gamma globulins were slightly elevated; in cirrhosis, beta and gamma globulins were significantly elevated; in the middle and late stages of obstructive jaundice, alpha 2 and beta globulins were elevated; in primary biliary cirrhosis, alpha 2, beta and gamma globulins were elevated.
