How to treat neovascular glaucoma

Clinically, neovascular glaucoma is divided into three stages: early glaucoma, open-angle glaucoma, and narrow-angle glaucoma.

In the early stage of glaucoma, intraocular pressure is normal, and only small new blood vessels appear in the iris and chamber angle. The most effective treatment measure at this stage is laser panretinal photocoagulation. Photocoagulation destroys the hypoxic retina, increases retinal vascular perfusion, interrupts the source of new blood vessels, and the new blood vessels shrink after panretinal photocoagulation. Panretinal cryosurgery can be used for patients with refractive media opacities. In the open-angle glaucoma stage, intraocular pressure increases and new blood vessels increase.

If vision is good, the refractive media is clear, intraocular pressure is controlled by medication, and miotic agents are avoided, panretinal photocoagulation is still the first choice. If medication cannot control intraocular pressure, surgery can be used, such as subscleral drainage tube implantation and valve implantation, filtration surgery, and ciliary body cryosurgery.

In the angle-closure glaucoma stage, the angle of the chamber is closed and the intraocular pressure is very high. You can try to use drugs to control intraocular pressure, and if conditions permit, perform panretinal photocoagulation and angle photocoagulation, and then use the above-mentioned surgical treatment. For patients in the late stage who have lost their sense of light and suffer severe pain, alcohol and chlorpromazine can be injected retrobulbarly to relieve the pain. Patients with corneal bullae or ulcers may choose to have their eye removed.
