Understanding Erythema Nodosum in Children

Erythema nodosum is an inflammatory skin disorder in which tender, erythematous masses (nodules) appear under the skin, usually on the shins but occasionally on the arms and other areas. Usually erythema nodosum is a phenomenon caused by certain diseases or drug allergies. It is more common in young people, especially women. It may recur months or years later. Bacterial, fungal, and viral infections can also cause erythema nodosum. Streptococcal infection is the most common cause, especially in children.

Sarcoidosis, ulcerative colitis, and various drugs such as sulfonamide antibiotics and oral contraceptives can also cause the disease. A variety of other infections and several types of tumors can also cause the disease. Treat any primary infection. If the infection is caused by streptococci, antibiotics such as penicillin or cephalosporin must be used. If untreated, the nodules will disappear in 3-6 weeks. Bed rest and nonsteroidal antipyretics can relieve pain caused by the nodules. Individual nodules may be treated with an injection of corticosteroids, and if people have multiple nodules, corticosteroids or potassium iodide tablets are sometimes used to speed pain relief.
