Moderate and severe patients often have physical and neurological signs, such as systemic developmental disorders; gigantism, dwarfism or morbid obesity; skull deformities; macrocephaly, pointed head, dolichocephaly, plagiocephaly, scaphoid head, etc.; in addition, there are finger and toe deformities, abnormal palmoplantar dermatoglyphics, widened distance between the eyes, ear deformities, etc.; as well as neurological signs such as motor paralysis, hypertonia, ataxia, speech disorders or deafness.

3. Extremely severe: IQ is below 20, extremely rare, accounting for less than 1% of mental retardation. They are born with physical and nervous system abnormalities and generally cannot learn to walk and talk, and can only make simple sounds like calling people. The senses are dulled and unable to avoid danger. Completely unable to take care of himself. Severe and extremely severe cases are equivalent to the "idiot" diagnosed in the past. Based on personality traits, they can be divided into the stupid, dull and slow type and the unrestrained, willful and irritable excitable type.
