1. What's wrong with a one and a half month old baby's bone alkaline phosphatase being very high?

There is no need to worry if the baby's bone alkaline phosphatase is high. Infants and young children are in a stage of rapid growth. High bone alkaline phosphatase cannot be used as a basis for the diagnosis of rickets. Instead, the level of 1,25-hydroxyvitamin D3 in the blood should be tested and vitamin D3 supplementation should be considered. Excessive calcium supplementation will cause calcium to combine with fat in food in the intestines to form calcium soap, leading to constipation.

2. Causes of increased alkaline phosphatase

Elevated alkaline phosphatase is mainly seen in: hepatobiliary diseases, obstructive jaundice, primary liver cancer, secondary liver cancer, leukemia, cholestatic hepatitis (significant increase in alkaline phosphatase), osteitis deformans, rickets, osteomalacia, hyperthyroidism and hyperparathyroidism, renal tubular acidosis, and hereditary hyperphosphatase. Fracture healing period, osteomalacia, rickets, osteocellular carcinoma, osteoporosis, etc. can also cause an increase in serum alkaline phosphatase. It is recommended to go to the hospital in time to find out the cause and receive symptomatic treatment.

3. Normal value of alkaline phosphatase

The reference normal value of alkaline phosphatase may vary depending on the test method or population, and the difference in the normal value range is also related to the methods and instruments used by each hospital. Under normal circumstances, the normal reference value of alkaline phosphatase is: 50~135U/L for women and 45~125U/L for men. Anything outside this range is considered high. High levels of alkaline phosphatase are more common in three groups of people: children in their growing years, pregnant women, and those in the fracture healing period.

4. Physiological factors of elevated alkaline phosphatase

1. During the physiological bone development period of children, the activity of alkaline phosphatase can be 1 to 2 times higher than that of normal people. Alkaline phosphatase is normal in osteoporosis.

2. Serum alkaline phosphatase levels may increase in growing adolescents, pregnant women, and those who have eaten high-fat foods or after strenuous exercise.

3. Pregnancy can lead to increased serum alkaline phosphatase levels. At nine months of pregnancy, serum alkaline phosphatase levels can reach 2 to 3 times the normal level.
